EXTENDED STAY FORM

The extended stay option is only available if the puppy has been paid in full by the age of 10
weeks. We will extend the stay of the puppy for two weeks at an additional fee of $50.00. During
the extended stay, the puppy will receive another dose of worming medicine and the second 5 in
1 round shot.

THE PUPPY MUST BE PICKED BY THIS DATE. THE fee of $50.00 is for the opportunity to
extend the stay of the puppy up to 2 weeks. No part of the fee will be reimburse if the puppy is
picked up before the extended pickup date.
